If you regularly take some type of treatment, you will be given a repeat prescription request form. About one week before you need more treatment, please send us your request using one of the methods listed below:
- Via the NHS App – Owned and run by the NHS, the NHS App is the most simple and secure way to access a range of NHS services on your smartphone or tablet. Easy to register now so that you can order your normal medications without leaving home.
- Online (Via Patient Access) – (If you are already registered for this service). You can view your current repeat medication and order the items you require.
- By repeat slip or on paper – Tick the items required on the repeat request form attached to your previous prescription and return it to the surgery.
- In person – You will be asked to complete a repeat prescription request form.
Please note – in order to avoid any potential for error, we require full details of the medication you require (drug name and dose). If full information is not provided we may be unable to issue your prescription.

Non-urgent advice: Notice
Please allow 72 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
Please note: if you require an item of medication which you have not requested for some time this may require a discussion with a member of our GP team before the item can be provided.

In order to sign up, patients will need to complete a registration form and will be required to provide documentation to verify their identity (further details are included on the registration form itself). Registration forms are available from Reception or you can download an electronic copy – see link below.
Once complete, you will need to bring your form in personally to reception along with your identification so that we can process your application.
