medication_10Our Dispensary is able to provide you with your medication if you live more than 1 mile away from your nearest pharmacy, otherwise you will be issued with a prescription to take to your choice of pharmacy. 

If you are prescribed any long term medications they will be available on repeat and you will be issued with a Yellow Card.

Repeat prescriptions can be requested in the following ways:

Please note – in order to avoid any potential for error, we require full details of the medication you require (drug name and dose).  If full information is not provided we may be unable to issue your prescription.

  • Tick the items required on your yellow card and return your card to the surgery.
  • In person - you will be asked to complete a repeat prescription request form. 
  • By using the online form on this page - you will receive an automated response acknowledging receipt of your request.
  • By fax * – 01487 832753

* If you are making your request by fax please remember to provide your full name, address and date of birth.

Please allow at least 2 working days for the processing of all repeat prescription requests.
